How many entries? If there are few words/sentences in the database the random selection of words could pick the same one multiple times.

I bet this is caused by phantom entries (search the forum for this term). Depending on who owns or created the course you could try to contact them here. Look for a thread named [Course Forum] name where name is the name of the course. You can also try to contact the creator here (tag them via @username) or send them a private message.

there were 5-7 entries, but only 1 had a problem
